Online students do not need to submit immunization records to register for courses. However, online students seeking a waiver for on-campus or hybrid courses must comply with University immunization policies to receive approval.
To register for on-campus or hybrid courses, you must submit an immunization form from your physician, or an exemption, along with a tuberculosis questionnaire. Processes and additional information are outlined below.
To request an exemption:
- Log in to using your 麻豆果冻传媒ID and password.
- Navigate to the 鈥淗ealth & Wellness鈥 card on your 麻豆果冻传媒ink dashboard.
- Select "Medicat Patient Portal."
- In the Medicat Patient Portal, select "Forms."
- On the Forms page, select 鈥淩equest for Exemption from Immunizations Form.鈥
- Save the PDF to your device and fill out the form, selecting all immunizations.
- Download for free and use the "Fill & Sign" feature under "All tools" to complete the form.
- Return to the Medicat Patient Portal and select "Upload."
- Navigate to the drop-down box at the bottom of the page to select 鈥淰accine Exemption Request Form.鈥
- With 鈥淰accine Exemption Request Form鈥 chosen, click 鈥淪elect File鈥 to upload your completed exemption form.
- Submit the file and allow for 1-2 business days for processing before your hold is lifted.
To submit your immunization records instead, follow these steps:
- Log in to 麻豆果冻传媒ink using your 麻豆果冻传媒ID and password.
- Navigate to the 鈥淗ealth & Wellness鈥 card on your 麻豆果冻传媒ink dashboard.
- Select "Medicat Patient Portal."
- In the Medicat Patient Portal, select 鈥淯pload.鈥
- Navigate to the drop-down box at the bottom of the page to select 鈥淚mmunization Record.鈥
- With 鈥淚mmunization Record鈥 chosen, click 鈥淪elect File鈥 to upload a photo or copy of your records.
- Submit the file and allow for 1-2 business days for processing.
To submit the tuberculosis questionnaire:
- From the top menu, select "Forms."
- Select "TB Screen Questionnaire."
- Complete and submit the form.
